Survived by mother, Judy Henslee of Franklin, TN; father, Mike Formosa; step-mother, Virginia Formosa both of CA; five siblings and several cousins, aunts and uncles. Memorial Services will be conducted 2:00PM Saturday, July 30, 2011 at Grace Episcopal Church, Spring Hill, TN. In lieu of flowers memorials may be made to St. Jude's Children's Research Hospital or the Muscular Dystrophy Association. Visitation will be one hour prior to the service at the church.
